Thank you so much for contacting me. I have attached a few of the pages related to patient documentation. I'm not sure if PT referred to physical therapy or patient. The documentation that I have outlined in my book is very general as it is intended to be applicable for PT's, OT's and SLP's. You may find that you need to adapt it to your patient population. In the very least, it will give you a guide to get started.
In addition to the basic soap note, I also include templates for an:
* Intake form
* Informed consent
* Release of information
* Plan of care agreement
* Record of contact
* Daily note
