I am interested in the coil bound book but wonder will the templates for documentation be available to me in a digital version so I can print and use or use on my computer? Also can a view a sample of the template for PT documentation as that is one of the things I am most interested in.

Hello!
Thank you so much for contacting me. I have attached a few of the pages related to patient documentation. I'm not sure if PT referred to physical therapy or patient. The documentation that I have outlined in my book is very general as it is intended to be applicable for PT's, OT's and SLP's. You may find that you need to adapt it to your patient population. In the very least, it will give you a guide to get started.

In addition to the basic soap note, I also include templates for an:

* Intake form
* Informed consent
* Release of information
* Plan of care agreement
* Record of contact
* Daily note
* Record of payment
